i wondered if midazalan is safer in cats with renal disease?

    oh ya

Also when i had my last consult with thd pain clinic i found out that GABA may not be metabolised properly if cat has had consistently low potassium. She has marginally low potaasium which shows up on her last 3 lab results.

Dose adjusted and supplement started and it seems effective

    hi

my cat gets a gaba liquid made by Summit. Apparenfly there are 2 liquid forms available. The Pain Service at Langford seem to be under the impression the Summit one is more palatable. Cat always looks like she swollowed an acid frog when i give it to her.

i have seen it given to hospitalised post blocked cats and post sub surgery

I have also given the powedered version mixed inro water to an aggressive cat with good results. the reason we did that was the cat was inventing new ways to kill us every second so he was difficult to handle or pill.

im not sure if this preparation modifies Gaba in any way. i could not find any info on that but stand by to be corrected

    Thank you Sara
Where I was we were measuring the bladder diameter accross and down so when I say a bladder is 8cm it means
8cm by 8cm so i guess one could estimate the volume using these measurements with the formula for volume of a sphere

    Pls see attached Bladder Measurements

I guess they developed this tool over years of being able to estimate fullness of bladders

I was also told not to use drip pumps for transfusions. But I have seen it done before. The patient underwent a spelnic removal and the vet used a drop pump (baxter) to transfuse. Not really sure why they did but I have always been told not to

    Hi Sarah

I locum so dont really get opportunities for M&Ms. I also dont think this would be flagged up as the general consensus seems to be this patient has a heart condition. As a locum it can be awkward to arrange discussions of this kind but i did mention the issue and just feel saying more night cone accross as pushy

oh thanks Scott. Perhaps the MSc will give me better knowledge to communicate with confidence and perhaps a bit of finesse. I admit i may have been less effective in my communication than i could have been. the vet was a very experienced GP and its really daunting when you feel like your telling someone of that magnitude something

Thank you Simon. It is hard and I am definately sensitive to trying not to offend my colleagues. I think im too gentle when i state these things. Its also difficult when your patient is not responding, you have no ecg and the situation becomes highly stressful. The patient in question did come round but the panic in the room was palpable. I have discussed this with other vets in conversation. i.e. not related to a patient and am just frustrated its happened to me twice now even though i know the theory. i also feel bad that i was ineffective in DOING more to prevent it at the time. now the patient is being referred to cardio for a heart condition and i dont think it has one. I think we just induced dysrhythmias under GA 🙁

Thanks for clearing that up Sara. Fab, and the glucose makes sense. I thought of that but maybe was overthinking it a bit.

I read somewhere this is not a reliable indicator in cats?

Its definately used in horses but where i worked we rarely did this and I do not think we had a lactate monitor. It was years ago so now Im just showing how ancient i am. Now I think they regularly do it in horse practice. We used to measure protein levels in a belly tap sample…so a Total solids on the fluids. Is this any use in small animals?

Sara Jackson, with regards to the glucose/lactate does the following indicate sepsis
peritoneal fluid = lower glucose than serum
    = higher lactate than serum

I assume the lactate indicates anaerobic activity of the bacteria. Can yohu remind mw why the glucos is loweR?

    Thanks
