PS Would you happen to know where I can find any instructions on how to implant a sensor in a dog? This owner may go for something like this, she finds it terrible that everyone on the FB diabetic dog group measures BG at home, and she can’t no matter what she tries.
Hi Scott, I just finished watching the DM segment- thanks for that, it was extremely informative. A few lessons with my case above: 1) am I overthinking the glucosuria? If it quacks, it’s a duck, and only ever a duck. Clinically, she seems ok.
2) Was initial treatment/work up correct? I don’t now, it was somewhere else. Notes/history was sparse.
3) The glucose curve is still giving me a headache, almost wish I hadn’t done it for this lovely client who unfortunately is limited with costs. Did I waste her money? Unacceptable.
The initial hypoglycaemia on the curve: a) did the owner (or partner) accidentally give insulin at home already? (I’ll make sure) b) is this still the insulin having an effect from the dose the night before? Should I trial a shorter acting insulin BID or a longer acting insulin SID? c) Should I convince the owner to invest in a sensor? She has tried everything to monitor BG at home but dog very uncooperative, and she can never make the dog bleed anyway if she is able to prick. d) The curve either tells me absolutely nothing, or it tells me that control is rubbish? But she only had half her normal dose….. FFS, what have I done. I wish I had finished your course first before I scheduled her BG curve…..

I am quite a vintage vet who studied in Germany, having graduated in 2004. I worked in Germany for a year and a half as a new graduate, and never wanted to work as a vet again after that experience. So, I did my doctoral thesis in atypical scrapie genetics, translated a few chapters in the König/Liebich anatomy book from German to English and then took a leisurely 6 years to write up and defend my thesis. During that time I also added to the world population by 2. I went back to work in 2013 at the PDSA after moving to the UK in 2008. Now I work in a small animal practice in North East England (imo the best area in the world) and have a lot to review/catch up on/remember from way back when. So, I will apologise now for my old fashioned ideas and questions. Please have patience with me. I am looking forward to hopefully being able to make a plan with my suspected endocrinopathy patients, or at least identifying them in the first place.
It’s funny that this is up, I had 2 dogs in the past year present very similarly- lethargic, brick red mucous membranes and with a high PCV, but no dehydration. Everything else normal. They were both older dogs. I googled, as you do, and polycythemia came up as a possibility. Unfortunately, I have no follow up on either dog, but one of the treatments listed was blood letting. This sounded the very opposite of elegant medicine, but would this be an ’emergency’ solution if the animal was struggling? Not to detract from the cat’s case, and curious to hear the outcome.

thank you SO much for your input. It is greatly appreciated. I actually did attempt a quick scan of the heart- I have a ‘new’ machine and was trying the cardiac probe for the very first time (no cardiac scanning table yet so was a bit challenging). I am not that experienced with cardiac scans, and I was running out of time trying for the proper views. It did look odd to me. I have made videos of the scan with my phone, I wonder if there was somewhere to post these as well? They are probably so rubbish, they may not be of any help or provide any additional information…. I am just really trying to do the best for this cat as well as for the owners. I might be able to post them on your FB page? Let me now if there is a way to add them to this discussion thread.
The lung sounds were harsh- there was definitely no wheeze, more like a soft crackle which was louder on inhale.
